This survey aims to identify the unmet health needs  reported by those of us living in the East New York community.  This vital feedback will promote a better understanding of the specific actions desired by us to promote opportunities for improved health and quality of life.  Without your involvement, the sustainable change we want and need will not take place.  The Rise Up campaign believes the creative and productive way to solve problems is by unlocking the community’s best resource, – us!
