Launch of the Rise Up East New York Community-Based Needs Assessment Survey

The community and it’s resources have a big effect on our health.  East New York may have some of the worst health statistics in New York State, but we cannot allow this any longer.  In order to prioritize next steps to meet the health needs of our community, the Rise Up campaign has developed a community-based needs assessment survey.

Our survey aims to identify the unmet health needs  reported by those of us living in the East New York community.  This vital feedback will promote a better understanding of the specific actions desired by us to promote opportunities for improved health and quality of life.  Without your involvement, the sustainable change we want and need will not take place.  The Rise Up campaign believes the creative and productive way to solve problems is by unlocking the community’s best resource, – us!
